I hereby consent to and agree to the following:

• I am over the age of 18, or I am the parent or legal guardian of the client.

• I understand that no provided/patient relationship is established through my participation in this session/event/class.

• I understand that Danielle Hibberd reserves the right to refuse a session/event/class or to end one at any time and will refund the client’s payment in full.

• I understand that Reiki practitioners do not diagnose conditions, nor do they prescribe or perform medical treatment, prescribe substances, nor do they replace the treatment of a licensed medical professional.

• I understand that it is recommended that I see a licensed physician or licensed health care professional for any medical or psychological ailment that I may have.

• I understand that this session is not meant to replace treatment by medical providers, but to complement them.

• I understand that Hope and Healing with Danielle does not bill insurance and payment is expected prior to the session.

• I understand that my session(s) will be kept confidential.

• No refund will be given once your session/event/class has been booked.
